Thank you for your interest in adapting materials developed by the Ontario Society of Physical Activity Promoters in Public Health (OSPAPPH).

This form is intended to gather information from members who wish to customize or modify existing OSPAPPH resources.

To support collaboration and continuous improvement, we kindly request that you share your adapted materials with OSPAPPH so we can share it back with our members.
